Health Information Director

The Health Information Director maintains the health information service for the facility in accordance with current laws and regulations and professional practice standards.

 

PRINCIPLE DUTIES:                      

  • Develops, organizes and maintains various control logs and files that are current. 
  • Ensures that all health information is protected and kept confidential in accordance with current laws and regulations. Protects all health records from loss, damage, or destruction prior to the statute of limitations.
  • Supervises the unit secretaries with health information responsibilities  
  • Coordinates ordering of all forms needed for the patient's health record
  • Ensures that audits are completed accurately for in-house records according to the established schedule.  Communicates with the DON and staff for the completion of deficiencies found with the audits.
  • Ensures that ICD-10 coding for diseases for residentsí diagnosis lists are done accurately and a timely manner or as needed.
  • Ensures that audits are completed accurately for discharge records and completes residentsí discharge records within 30 days. 
  • Maintains a filing system for both in-house and discharged residents to facilitate record retrieval and access.  Coordinates filing of records and reports into the residentsí health records in a timely manner with the unit secretaries and health information assistants.
  • Coordinates charts requests for copies of patientsí health records.
  • Assists with Quality Assessment/Assurance and other committee meetings; assists with Quality Improvement studies under the supervision of the Administrator, Director of Nursing and/or Health Information Consultant. 
  • Attends interdepartmental meetings as directed by the administrator. 
  • Participates in on-going in-service training related to health information, residentsí rights, etc. 
  • Supports software for clinical records.
  • Exhibits excellent customer service and tact when dealing with physicians, residents, and families.
  • Other duties as assigned by supervisor.

 

QUALIFICATIONS:

  • Previous experience in a skilled nursing facility as a health information or medical records director.
  • Knowledge of medical terminology.
  • Computer knowledge and typing/data entry skills; knowledge of Microsoft Office.
  • Knowledge of ICD 10 coding.
  • Excellent written and oral communication skills.
  • Excellent customer service skills and telephone etiquette.
  • Must be able to concentrate with frequent interruptions.
  • Dependable, accurate, self-motivated, independent, neat and organized.
  • High school graduate, or equivalent.

 

CRITICAL FUNCTION:

  • This position is essential to the safety of our residents and staff; and to the operation and function of the community. Employee may be recalled to report to work for an emergency, unusual occurrence, resident care need, visit from regulatory/governing agencies or any other event deemed necessary by the Executive Director, Administrator, and designee.

 

PHYSICAL REQUIREMENTS/WORKING CONDITIONS:

Must be able to sit at a desk for up to six hours per day; must be able to stoop, bend, stretch, and squat to access files and various documentation. Must be able to occasionally transport residents via wheelchair, pushing up to generally 200 +/- lbs. Must be able to lift up to 35 lbs occasionally. When lifting over 35 lbs, you are required to ask for assistance. Occasionally push and/or pull wheeled carts and equipment generally weighing up to 200 lbs. May be exposed to communicable diseases including HIV and Hepatitis B viruses; industrial and household chemicals; offensive odors; and combative behavior.
